New Patients

We are currently accepting new patients with a wait time of three weeks or less.

NEW PATIENT REGISTRATION FORM

INTAKE FORMS are completed prior to scheduling with a provider. If you are unable to complete the new patient intake forms online through our portal, please contact us at (775) 506-2606.

LATE ARRIVALS If you arrive more than 10 minutes late to your appointment, we will need to reschedule your appointment.

COPAYS deductible and/or coinsurances will be collected at the time of check-in. Please remember it is your responsibility to understand your insurance benefits. All non-medicaid patients are required to maintain a card on file for billing purposes.

New Patient Scheduling Process:

Step 1 - Start by registering with us - use the “New Patient Registration Form” button above

Step 2 - Once we receive your registration form, we’ll email you an invitation to create a patient account on our portal. Please allow up to 24 hours to receive the invitation.

Step 3 - Once you’ve created your account login, you will receive an email notification that you have forms to complete in your portal account. These are the new patient intake forms that must be completed before we can schedule you with a provider. ALL FORMS must completed and signed.

Step 4 - Once you have completed and submitted your forms, please call our scheduling team at (775) 507-2606 to schedule your appointment. If we do not answer, please leave a voicemail and we will return your call. New Patient appointments must be scheduled by our office staff - please do not attempt to schedule your new patient appointment through your patient portal account.

We ask that you give at least 24 hours notice if you cannot make your scheduled appointment. All appointments not cancelled more than 24 hours prior to your scheduled time will be charged a no-show fee of $50 (subject to change). This fee is non-payable by insurance, and is the responsibility of the patient. No-show fees must be paid before we can schedule a future appointment for you.
